Avg 3 – 9 patients per day with extended 1:1 time
Nursing home and assisted living visits required
Team-based care — IDT meetings every morning
Call is 1:7 — mostly telehealth, routed to RN first
Acute and episodic visits included
Adults only — IM or FM with adult patients accepted
Hospital privileges required
